New Club Information Report

Each new club will complete a Club Information form and send it to Youth Ministries at time of registration. A club should complete this form only once, unless the club disbands for a period of time and then starts up again.

Attendance & Activity Report

There are three Attendance and Activity reports during the year. They reflects accomplishments of the previous 3 month period, status of club and activities planned for the next 3 month period.

Year-End North American Division (NAD) Report

A great deal of emphasis is placed on the NAD year-end report. This information is used by the NAD Youth Ministries Department, Union, and Conference personnel to determine what is needed to strengthen the Adventurer program. It is vital that your information be as accurate as possible. If your staff keeps weekly records of parent and child attendance, awards earned, family network meetings, and investitutes, this report is easy to complete at the end of year after investiture. This report is due July 31.
